To register a New Student, Utah State Law Requires:

1. COMPLETE IMMUNIZATION RECORD

Minimum Requirements:

1 Varicella (Chickenpox Vaccine)

2 Hepatitis A

2 MMR

3 Hepatitis B

4 Polio

5 DPT

2. ORIGINAL BIRTH CERTIFICATE

3. PROOF OF RESIDENCY IN OUR BOUNDARIES* – Any document with your name attached to an address in our boundaries where you reside. Ex: Bill, Checkbook, rental agreement, etc.)

4. SOCIAL SECURITY CARD

*If you would like to pick up registration forms, please make sure that you bring your proof of residency at that time.

Only the legal guardian of a child may pick up registration forms and register that child for school.
